We are called Stevenson Family Dentistry because we really enjoy serving all family members. Located in Jacksonville, Florida, we practice family dentistry as well as orthodontics for both children and adults.

For this reason, our office meets and surpasses all OSHA (Occupational Safety and Health Administration) and CDC (Center for Disease Control) standards. We are confident that you will feel right at home in our office as we welcome all patients as if they were family.

Payment Policies

Dr. Stevenson’s staff will be happy to assist you in filing insurance claims, however we are not an in-network provider for any insurance company. Please bring your insurance card to your visit. All co-payments are collected at the time services are rendered. In addition, we accept all major credit cards and cash. Payment options can be discussed at the time the treatment plan is presented. Be sure to ask about all the details during your visit.

Insurance Claims and Financial Responsibility

We will be more than happy to submit all insurance forms for you and help you recover the most from your benefits. We will do everything we can to help you afford the treatment you need and want. For patients who require major work, a complete payment plan is designed with an appropriate payment schedule. Forms of payment accepted by the office are cash, check and any major credit card.

Appointment Policies

If you would like to make an appointment, please call us directly at (904) 268-4466. We are available by phone Monday 8AM to 5PM, Tuesday Closed except 1st and 3rd Tuesday 2PM to 7PM, Wednesday 8AM to 5PM, Friday: 8AM to 5PM. We are closed for lunch from 1PM to 2PM. To make an appointment outside of our normal business hours, click here and fill out the form. We will contact you to schedule an appointment.

If you are unable to keep an appointment, we ask that you kindly provide us with at least 24 hours’ notice. We ask for this advance notice so that we can offer this appointment to another patient.
